Calculates glucose infusion rate from dextrose concentration, fluid rate, and patient weight

The glucose infusion rate (GIR) calculator determines the rate of intravenous dextrose delivery (mg/kg/minute) to maintain appropriate blood glucose in neonates and infants with persistent hypoglycemia despite enteral feeding or glucose gel administration. This calculation guides IV dextrose titration in preterm infants, growth-restricted infants, and those with hyperinsulinism or other causes of severe hypoglycemia. GIR requirements typically range from 4-8 mg/kg/minute in normal neonates but may exceed 15 mg/kg/minute in severe hyperinsulinism.
